By completing this form for a recurring donation I understand that I am authorising CareFlight (Debit User 089741) to debit my account I have nominated with the amount shown above, on the due debit day monthly, subject to the Direct Debit and Credit Card Service Agreement terms and conditions until revoked by me. I agree that this debit or charge will be made through the Bulk Electronic Clearing System (BECS) or associated credit card scheme.
